Direct Primary Care (DPC) is a healthcare model that focuses on building strong provider-patient relationships by removing the complexities of traditional insurance. Patients pay a flat monthly or annual membership fee that covers a range of primary care services, including office visits, preventative care, chronic disease management, and basic lab work.

We do not accept any insurances. All members are responsible for their monthly membership fee regardless of insurance status. If you have an HSA or FSA, you may be able to use these accounts to cover your membership. It is your responsibility to check with your specific company regarding their requirements

Absolutely NOT! In fact, we encourage members to maintain insurance coverage, at least catastrophic, in order to cover the expense of sudden and unanticipated health events including hospitalization. Your insurance may also be used to pay for services provided by outside vendors for imaging, labs, consultations, brand name medications and procedural contraceptives, and vaccines.

You will be charged a registration fee and subscription fee at the time your visit. At your first visit we will answer any outstanding questions you may have about the practice and confirm that Chamber of Wellness is a good fit for you and your family. If not, you will not be charged for the first month and, whenever possible, recommend other providers in the area that can accommodate your families needs.

We accept patient from the age of 12-64.

You are allowed to bring one family member or friend to your visits to provide interpretation for you. If you do not have anyone to interpret for you, we will make available interpretation services.

There are no barriers to establishing care with Chamber of Wellness based on citizenship. Our aim is to increase access to care for all persons in our community.

Absolutely! Our Electronic Health Record is designed to collect and send data across platforms to ensure we are up to date on your medical journey and able to communicate easily with your entire care team.

Established patients with a need for after hours services will be able to schedule visits outside of our normal business hours with practice approval. In addition to scheduling visits, our members have the luxury of text/call/email access to determine the acuity and appropriate timing for care.

Patients are set up on autopay by credit/debit card including your Health Savings Account card or credit card. Payment will default to reoccur on the 15th of each month. For point of care sales including in house medications and labs you will be able to pay with your credit/debit card on file.

Urgent Care Visits provide fast, convenient medical care for minor illnesses and injuries. Designed for individuals seeking immediate attention, these visits ensure timely treatment, helping patients get back to their routine quickly.
Common Conditions Treated:
Illness: cold, flu, sore throat, sinus infections, ear injections, strep throat, and pink eye.
Injuries: minor cuts, scrapes, burns, sprains, and strains.
Skin Conditions: rashes, insect bites, poison ivy, and mild allergic reactions.

Lab Work
Lab Work is an essential part of healthcare, offering accurate and timely insights into your overall health. Our on-site lab services provide convenient access to a variety of diagnostic tests, ensuring fast results and seamless care.
Types of Lab Tests Offered:
Annual/Wellness Labs: complete blood count (CBC), chemistry panel (CMP), cholesterol panel, thyroid function tests, hemoglobin A1C, and vitamin d level.
Urine tests: urinalysis and pregnancy tests.
Infection Testing: strep throat, flu, covid-19, and urinary tract infections (UTIs).
Tuberculosis (TB Quant)
ABO/Rh Type
Sexually Transmitted Disease (STD) testing: HIV, syphyllis, herpes simplex
